Community Action Network (CAN)

The Community Action Network (CAN) is a partnership of agencies, community-based organizations, and community members that work together to achieve common goals for community improvement, and coordinate services to improve health outcomes for all community residents. 

​

The CAN is a meeting for the community’s voice to be heard in efforts to improve the health of infants, mothers, and families. 

​

The CAN encourages health care, social service providers, and others to integrate services and address the different areas of life such as where we live, go to school, work, and other factors that affect health outcomes.

Expectations of Participation
  • Attend Monthly Meetings which are currently the 2nd  Wednesday of the month from 1:00-3:00 p.m.   (Virtual options are available when appropriate).

  • Participate in the discussion

  • Serve on one of the sub-committees

  • Attend trainings on Cultural Relevance and Collective Impact

  • Be available to lead a discussion with the support of the partners and navigators

  • Share your experience via surveys and possible interviews

  • Invite others to participate
